Continuing Education Forms


Continuing Education Program Approval

All continuing education (CE) programs must be approved by the Board prior to being offered for credit.

To request approval, applicants must submit a Continuing Education Program Application Packet, all required supporting documentation, and the $30 application fee.

Preferred Submission Method (Online)

Online submission is preferred to facilitate faster review and approval. Applicants may complete the application, upload all required documents, and submit payment through the Board’s online system.

Alternate Submission Method (Mail)

Applicants who are unable to submit their application online may download the application packet and mail the completed materials, along with the $30 application fee by check, to the Board office.


Request for Approval of a Continuing Education Course by a Licensee

For licensees who wish to have a particular Continuing Education course they have taken or will be taking approved for continuing education credits, please complete the application (provided in the link below) and forward application and the $25 fee to the Board office.
