Continuing Education Program Application Packet
To have a program approved by the Board, the applicant must submit the Continuing Education Application Packet, the additional required forms, and the $30 application fee to the Board Office either by submitting the online application and payment form, or by clicking on the link below and mailing the completed application and fee to the Board office.
