Board Agenda Request

Submit this form to the Board office to receive a copy of agenda by mail or email prior to each Board meeting.


Change of Address/Name

Complete this form if there is a change in EITHER your address or name, as recorded by the Board. The $30.00 fee applies only to name changes with an updated copy of the license.


Request for License Verification

Complete this form to request a verification of your license or certificate. Submit $20.00 per request for verification.  To pay online, contact the Board Office in advance.


Application Release - Outside party

Below release should be signed by applicant if person other than applicant must discuss status, changes and other application information.
